Introduction: A high mortality rate is associated with hip fractures in the elderly. This because their bones are osteoporotic implants having less hold and there more co-morbidities Osteosynthesis proximal femoral nail (PFN) features advantages of rotational stability head-neck fragment. However, use technically ambitious accompanied by some risks error, which can lead to failure. study aims understand technical difficulties related PFN methods mitigate them radiological indicators for successful outcomes PFN. Methods: Our analyze parameters as outcome intertrochanteric fixed using also factors responsible intraoperative conversion dynamic screw (DHS). a prospective, observational conducted from January 2020 December 2020, on all patients who were planned be treated consented part group at our institute. includes 99 cases classified according AO (Arbeitsgemeinschaft für Osteosynthesefragen)/Association Study Internal Fixation (ASIF) Evan’s classification systems followed postoperatively regular intervals up one year. According Chang's reduction quality criteria (CRQC), two have CRQC 1 (poor reduction), 11 2 (acceptable 39 3 28 4 (excellent reduction). Conclusions: Though challenging, proper guidelines technique, it gives excellent results. Most importantly, nonvarus insertion accurate placement lag screws crucial outcome. Biomechanically stable closed, percutaneous, or open means, key treating unstable successfully.
